Rule 1005.
Caption of a Petition; Title of the Case
(a)
Caption and Title; Required Information.
A petition’s caption must contain the name of the court, the title of the case, and the case number (if known). The title must include the following information about the debtor:
(1)
name;
(2)
employer-identification number;
(3)
the last 4 digits of the social-security number or individual taxpayer-identification number;
(4)
any other federal taxpayer-identification number; and
(5)
all other names the debtor has used within 8 years before the petition was filed.
(b)
Petition Not Filed by the Debtor.
A petition not filed by the debtor must include all names that the petitioner knows have been used by the debtor.
